Redesigning care pathways, removing bottlenecks and implementing best practice can dramatically improve the patient experience, increase capacity and even reduce costs.

Ashfield In2focus has devised (in collaboration with Pharmaceutical company sponsor) and implemented service re-design programmes. These programmes are devised and delivered using the NHS Institute for Innovation and Improvement methodology. Our Nurse Advisors have received training in service re-design and facilitation. In each programme the Ashfield In2Focus Nurse Advisor will facilitate or undertake the following steps:

Stage 1 – Referral of Secondary Care Unit  

Stage 2 – Initial Meeting

Stage 3 – Service Analysis  

Stage 4 – Service Feedback

Stage 5 – Process review at one month (or as agreed with unit)

Stage 6 – Re-assessment at 3 months (if required)

Stage 7 – Programme feedback
